Coeliccia cyanomelas (Blue forest damsel)
Habitat: larvae found under stones or leaf litter in clean and shaded streams
General appearance: body medium sized, probably up to 18mm long; head pentagonal, pale spots across head between eyes; labium kite-shaped with 4 setae on each side of prementum; legs slender with alternate pale and dark bands; three caudal gills sword-like, proximal portion longer and darker with spines along margins, distal portion shorter with pale pointed tip
